Financial Information

For your convenience, we accept Visa, MasterCard, American Express and Discover cards. We deliver the finest care at the most reasonable cost to our patients, therefore, payment is due at the time of your consultation for all consultation fees (x-rays, CAT Scans, etc.) and payment in full for all surgical procedures is collected fourteen (14) days prior to your surgery date.  Payment in advance allows us to order the supplies needed for your surgery as well as assure appropriate stafffing.

Our office offers treatment financing through Care Credit and Chase Health Advance.  Please ask one of our staff members for information about these excellent interest free financing plans.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will be happy to submit an insurance form on your behalf so that you may receive reimbursement for any covered charges from your insurance company.
